In compliance with City Ordinance No. 2001-103, all insurance companies are invited to apply for accreditation with the Office of the City Mayor for authority to sell Comprehensive General Liability (CGL) Insurance to Business License/Permit applicants in Makati City.

Application forms are available at the Office of the City Administrator, 20/F Makati City Hall. Deadline of submission of application forms and requirements shall be on December 2, 2009.
