Only Passport Applicants who have submitted Duly Accomplished Passport Application Forms together with all the Requirements on or before 24 October 2008 at the Office of the City Administrator will be processed on 08 November 2008.  All Applicants will be given a time appointment upon submission of Passport Applications.

 

No walk-in will be entertained on the processing date.

1.      Must be bonafide Makati constituents at least 15 years old and above

2.      Must Present Valid ID¿s of residency or Bgy. Clearance

3.      Fill up an enrollment form/Registration form

Must submit the copy of the following

Voter¿s registration receipt (VRR), identification card with residency address, Endorsement letter from Bgy. Captain & 2 recent copies of 1 x 1 pictures
