SENIOR CITIZENS' MID-YEAR CASH INCENTIVE 2018

GUIDELINES ON THE DISTRIBUTION OF THE 2018 MID-YEAR CASH INCENTIVE FOR MAKATI CITY SENIOR CITIZENS (for Pilot Barangays)

All Senior Citizens issued with Blu Card from June 2002 to April 13, 2018 shall be entitled to receive the mid-year Cash Incentive for the First Batch. Those issued with Blu Card on April 16, 2018 to June29, 2018 shall receive the same Cash Incentive for the Second Batch.

Blu Cardholders shall receive the following mid-year Cash Incentive amounts corresponding to their age categories:

60-69 Years old : Php 1,500.00
70-79 Years old : Php 2,000.00
80 Years old & above : Php 2,500.00

The standard mode of receiving the Cash Incentive is in the form of Cash. However, beginning this year in selected pilot Barangays, Blu Cardholders may receive their Cash Incentive either in CASH or CHECK, subject to the following guidelines. In this regard, the Blu Cardholder shall accomplish a form indicating his/her preferred mode of receiving the Cash Incentive. Blu Cardholders who fail to accomplish and submit the said form to MSWD on or before June 22, 2018 shall automatically receive their Cash Incentive in the form of cash.

OPTION 1: CASH

1. Blu Cardholders must PERSONALLY CLAIM their mid-year Cash Incentive on the scheduled date and venue to be announced later.

2. HOWEVER, Blu Cardholders who are either a.) bedridden, b.) disabled, or c.) confined in the hospital may authorize ANY of the three (3) designated beneficiaries indicated in their Blu Card only to claim their mid-year Cash Incentive.

Official Authorization Form (1) issued by the Makati Social Welfare Department (MSWD) should be properly accomplished, signed by the Blu Cardholder and his/her authorized representative, attested by the Chapter Chairperson of the Barangay Council of Senior Citizens or the Punong Barangay where the Blu Cardholder resides and duly approved by the Head of MSWD.

If the Blu Cardholder is confined in the hospital, a medical certificate signed by the attending physician indicating the current hospital confinement of the same Blu Cardholder should be attached to the Authorization Form.

The authorized representative should present and submit two (2) photocopies of any valid government ID upon claiming the mid-year cash incentive. (Attached herewith is the list of valid government IDs which the city government shall honor.)

3. On the other hand, Blu Card holders who are out-of-town at the time of distribution are NOT ENTITLED to receive the mid-year cash incentive. Any authorization issued for this purpose shall not be honored. Please take note that similar to the Guideline 1 above, this policy has been in effect since the beginning of the Cash Incentive distribution feature of the Blu Card Program.

4. Designated beneficiaries of the Blu Cardholders who passed away on the cut-off date from April 16, 2018 onwards shall still be entitled to claim the 2018 mid-year Cash Incentive, other than the burial assistance.

5. The Senior Citizen’s National ID card, also known as the White Card, or any other Senior Citizen’s card shall be honored, provided that the Senior Citizen claims his/her Cash Incentive personally.

6. A Blu Card in any of the following conditions shall be confiscated and replaced at the MSWD after claiming the Cash Incentive:

a. Laminated Blu Card without the signature of the holder.
b. Torn Blu Card.
c. Blu Card ID number that is covered by photo of the holder.
d. Blu Card issued without beneficiaries.
e. Blu Card with corrections, erasure/s or alteration/s.

7. The Blu Cardholder or his/her designated beneficiary shall be given five (5) working days only to claim the Cash Incentive. On a designated date/s, the Cash Incentive distribution shall be conducted in the Barangay. In case the Cash Incentive is not claimed during the scheduled distribution in the barangay, the Blu Cardholder/designated beneficiary may claim it at the Cash Division, 3/F Makati City Hall Building I, J.P. Rizal Ave., Poblacion, Makati City. Otherwise, it will revert back to the general fund of the City and deemed forfeited.

OPTION 2: CHECK

1. Senior Citizen’s Cash Incentive under this option shall be paid through Check issued by the Land Bank of the Philippines. The Blu Cardholder taking this option must have an existing bank account under his/her name. The Check shall bear the full name of the Blu Cardholder Senior Citizen as Payee.

2. The Blu Cardholder or his/her authorized representative must claim the Check for the Senior Citizen’s Cash Incentive on the scheduled date and venue to be announced later.

Please take note that similar to the Guideline 1 above (i.e. for receiving Cash Incentive in Cash), this policy has been in effect since the beginning of the Cash Incentive distribution feature of the Blu Card Program.

4. The Senior Citizen’s National ID card, also known as the White Card, or any other Senior Citizen’s card shall be honored, provided that the Senior Citizen claims his/her Check personally.
5. A Blu Card in any of the following conditions shall be confiscated and replaced at the MSWD after claiming the Cash Incentive:

a. Laminated Blu Card without the signature of the holder.
b. Torn Blu Card.
c. Blu Card ID number that is covered by photo of the holder.
d. Blu Card issued without beneficiaries.
e. Blu Card with corrections, erasure/s or alteration/s.

6. The Check can be deposited to the bank account of the Senior Citizen only.

7. The Check shall be crossed and for deposit only.

8. The Check cannot be encashed.

9. The Check must immediately be deposited within a period of thirty (30) days from date of issuance. In case the Check is not deposited within a period of thirty (30) days from date of issuance, it shall be automatically forfeited and the Cash Incentive reverted back to the general funds of the City. Along with this action, the succeeding senior citizen Cash Incentive shall be automatically given in the form of cash, subject to the abovementioned guidelines in Option No. 1: Cash.

10. In case of loss, destruction, and/or expiration of the Check, it cannot be re-issued.

11. The Blu Cardholder or his/her designated representative shall be given five (5) working days only to claim the Check for the senior citizen’s Cash Incentive. On a designated date/s, the distribution of Checks shall be conducted in the Barangay. In case the Check is not claimed during the distribution in the barangay, the Blu Cardholder/designated representative may claim it at the Cash Division, 3/F Makati City Hall Building I, J.P. Rizal Ave., Poblacion, Makati City. Otherwise, the Check will be deemed forfeited and the Cash Incentive reverted back to the general fund of the City.
